I hereby acknowledge that I have received a copy of the conflict of interest policy of Ohio Tri-County Food Alliance, have read and understood it, and agree to comply fully with its terms and conditions at all times during my service as a board member. I further understand Ohio Tri-county Food Alliance is a charitable non-profit and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes.

 If at any time following the submission of this form I become aware of any actual conflict of interest, or if the information provided below becomes inaccurate or incomplete, I will promptly notify the Chair of the Board.
